Welcome to the Carbondale Nursing and Rehabilitation Center. The door is always open, and we are always happy to see you, whether you are a new resident just coming in or family and friends coming to visit.

Once you pass through the front doors, you will immediately notice some of our security measures. The inner door is always secure. An intercom is present so that you can speak to our receptionist and ask to come in.

Once you've been granted access, you will discover an elegant foyer, where you can sign in and speak to one of our receptionists.

Our staff is always glad to help you, and have any information you might need at their fingertips.
